Voluntary Recall Notice for iVIZIA^® Sterile Lubricant Eye Drops, Povidone 0.5%, 10mL
Thea Pharma Inc. is voluntarily recalling five lots of iVIZIA® Sterile Lubricant Eye Drops, 10mL at the retail level in the United States. This recall is being conducted due to Current Good Manufacturing Practices deviations observed by the U.S. Food and Drug Administration during an inspection of the manufacturer. Only the specific five lots listed below are included in the recall.

How to check whether your eye drops are included in the recall?

Check the lot number and expiration date information (“LOT/EXP”) on the side of the bottle or the top of the box. This will show the lot number and expiration month/year. If the lot number matches one of the lots listed above, your product is included in the recall. If it does not match, your product is not included in the recall, and no further action is needed.

What should you do if your eye drops are included in the recall?

If you have iVIZIA^® Sterile Lubricant Eye Drops, 10mL from one of the lots included in the recall, stop using the product. Please contact Customer Service at 1-800-240-9780 (Option 7) or info@ivizia.com for instructions on how to return the product.

If you would like a replacement, you will need to return the bottle or box showing a lot number included in the recall. Once your return is confirmed, Customer Service will provide additional information about the replacement process.

Additional information

This recall applies only to the five identified lots of iVIZIA^® Sterile Lubricant Eye Drops, 10mL. No other iVIZIA^® eye drops, eye gel, wipes or cleansers, in sample or commercial packaging, are included in this recall.
