1.    From healthcare providers only:

  • By which you may be personally identified, such as name, postal address, e-mail address, telephone number, purchase history, your National Provider Identifier (“NPI”) and your IP address (collectively, the “healthcare provider personal information”);
  • Healthcare provider’s primary specialty, if applicable.
  • Details about your medical practice, if applicable.
  • That is about you but individually does not identify you (unless you provide us information on your identity), such as your location, and such other information you provide to us when you use our Website.
  • Through Google Analytics (more on this below) and Meta Pixel (more on this below) that is about you but individually does not identify you, such as names of the pages you accessed, the browser and the operating system you used, the date and time of access, search engines you used, the equipment you used to access our Website and usage details such as traffic data, logs, and other communication data, resources accessed or used on our Website.

The following are the ways we collect information from you:

  • Directly from you when you provide it to us.
  • From third parties, for example, our service providers, including but not limited to Google LLC (“Google”), Meta Platforms, Inc. (also known as Facebook and hereinafter referred to as “Meta”), and GEODIS.

  • By which you may be personally identified, such as name, postal address, e-mail address, telephone number and purchase history (“non-healthcare provider personal information” and together with healthcare provider personal information, collectively, “personal information”);
  • That is about you but individually does not identify you (unless you provide us information on your identity), such as your location, and such other information you provide to us when you use our Website.
  • Through Google Analytics and Meta Pixel that is about you but individually does not identify you, such as names of the pages you accessed, the browser and the operating system you used, the date and time of access, search engines you used, the equipment you used to access our Website and usage details such as traffic data, logs and other communication data and resources accessed or used on our Website.

The following are the ways we collect information from you:

  • Directly from you when you provide it to us.
  • From third parties, for example, our service providers, including but not limited to Google LLC (“Google”) and Meta Platforms, Inc. (also known as Facebook and hereinafter referred to as “Meta”).

Google Analytics, Google Ads and Remarketing

We use a tool called “Google Analytics”, a web analytics service provided by Google to monitor the performance of our Website and collect information on your use of our Website. We also use Google Ads for remarketing and re-engagement of audiences which allow us to reach people who previously visited our Website.  Google may use cookies and/or device identifiers to serve you ads on various sites on the Internet based on your past visits to our Website. You may opt out of the use of this cookie by visiting Google’s Advertising and Privacy through this link or by visiting Google Ads Settings accessible through this link. We note that we only have access to de-identified information being collected by Google through these tools and we do not have control over such collection or processing.

Meta Pixel

We also use a tool called “Meta Pixel”, a small piece of JavaScript provided by Meta to monitor the performance of our Website and collect information on your use of our Website.  We use the Meta Pixel to reach people who visited our Website, optimize our ads across the Internet (e.g., creating custom audiences of our visitors), and enable us to obtain reports and insights on the results of such ads.   • To carry out our obligations and enforce our rights arising from any contracts entered into between you and us, including but not limited to processing your requests, purchases, transactions, and payments, and preventing transactional fraud (including exchanging information with other companies and organizations for the purposes of fraud protection and credit risk reduction); to enforce or apply our general terms and conditions of sale and other agreements, including for billing and collection purposes.
  • To provide you with support and to respond to your inquiries, including to investigate and address your concerns and monitor and improve our responses.

By submitting your personal information to us you agree to the transfer, storage, and processing of your information in a country other than your country of residence including, but not limited to, Switzerland and France.

  • Promotional Offers to HealthCare Providers only from Company. If we collected healthcare provider personal information and you do not wish to have your email address/contact information used by Company to promote our products, or receive educational materials about our products, do not check the relevant box that may be located on the form on which we collect your data.  You can also send us an email stating your request to info@ivizia.com. If we have sent you a promotional email, you may send us a return email asking to be omitted from future email distributions or unsubscribe by using the link on such email. This opt out does not apply to information provided to Company as a result of a product purchase, warranty registration, or other transactions.

Nevada residents who wish to exercise their sale opt-out rights under Nevada Revised Statutes Chapter 603A may submit a request to this designated address: info@ivizia.com. However, please know we do not currently sell data triggering that statute’s opt-out requirements.

We may not accommodate a request to change the information if we believe the change would violate any law or legal requirement or cause the information to be incorrect (in the case of a change).  We may deny a request for deletion if retaining the information is necessary for us to comply with applicable laws, rules, regulations and our other legal obligations (including the compliance with the FDA rules, regulations and guidance documents and completion of transactions), maintain and enhance our Website, address security issues, or make use of the information that are compatible with the context you provided it to us.
